JOB DETAILS

Job Function:

• Ability to exhibit critical thinking, effectively organize/prioritize accordingly, and implement evidence based care.      
• Providing, arranging for, teaching and supervising medical social services through the process of assessment, planning, implementation and evaluation
• Providing advisory service to other professionals who share responsibility for patient care and participating in the development of the Medical Social Service Program.
• Inputting all confidential documentation into EMR system
• Ability to travel to patient's homes

Job Requirements:

• State Licenses: Massachusetts
• Must have LICSW or LSW with home health and traveling experience
• 1+ years current experience as a Medical Social Worker in a healthcare setting
• Master's or Doctorate degree required
• BLS through the American Heart Association

• Active and valid driver license

• Community Health experience preferred
