JOB DETAILS

Job Function:

• Provider will be focused on LTC ISNIP (Institutional Special Needs Plan) patients  
• Monday-Friday 9am-5pm or 8:30am-4:30pm schedule (35 hours considered full time)
• Long term care and post-acute care provided at facility, but this role is primarily focused on long term care patients
• Patient panel of 166 total at the facility and split between 3 NPs is responsible for 75-80 at least once a month as needed with family outreach once a month - once fully staffed would be around 55
• Responsible for rounding, diagnosing, prescribing, death certificates and risk assessing diagnosis
• Work with dietitians, nursing staff, rehab staff
• Work with established team of physicians, NPs, nursing and therapy staff
• Nationally recognized facility with emphasis on research, education and a high quality of care  
• Schedule includes on-call sessions every 4-6 weeks for a FULL week of call from Mon 5pm to following Tues morning (all via phone) - paid for this

Job Requirements:

• Active NY License
• Nurse Practitioner (Adult or Family) Board Certification
• 2+ years of NP experience
• Experience with geriatric population, long term care or post-acute care desired
