The largest not-for-profit Program of All-Inclusive Care for the Elderly (PACE) in the nation is looking for a Palliative Care Nurse Practitioner Supervisor for their location in Queens. Job Function:

  • Manages participants with complex care need including frailty, multiple chronic illnesses and end of life care.
  • Conducts thorough assessments of members identified as having complex and Palliative Care needs including but not limited to: accurate history and physical assessment; medication reconciliation; goals of care identification, psychosocial/spiritual  needs assessment; symptom management identification, plan, and implementation of appropriate medical interventions.
  • Determines and documents appropriate follow-up plans and communicates these plans with member and caregivers.
  • Orders, provides oversight of, and coordinates delivery of prescriptions pertaining to the management of advancing disease processes for members enrolled in the palliative program and on the end-of-life panel.
  • Monday to Friday 8:30-5:30pm schedule (40 hour work week)


Job Requirements:

  • NY State license, DEA, CSR (active or applied preferred) but not required
  • Masters as an Adult or Family Nurse Practitioner
  • Board Certification Required
  • Previous experience in geriatrics, home care or care management required
  • 2+ years of experience in palliative care or hospice required
