The largest not-for-profit Program of All-Inclusive Care for the Elderly (PACE) in the nation is looking for a Nurse Practitioner (ANP or FNP) for their locations in Queens.


Compensation : $145000 - $155000

Job Function:

  • Will perform annual wellness assessments on all assigned site community participants and reflects an understanding of the complexity and multiple comorbidities of the elderly
  • Monday to Friday 8am-5pm schedule (40 hour work week)
  • Will be based out of clinic but may need to go into the field and conduct home visits, but there are home aides that do this  
  • Responsible for revieing patients' medical history every 6 months and assesses those that were hospitalized or sent to ED
  • Will be responsible for identifying active diagnoses and chronic problems or conditions to be used in Care Management and active medical management of treatment and designed interventions
  • Responsible for supervising the care plan and delivery by each provide  


Job Requirements:

  • NY State license, DEA, CSR (active or applied preferred) but not required
  • Masters as a Physician Assistant  
  • Board Certification Required
  • Previous experience in geriatrics, home care or care management required
