Immediate opening for a patient care coordinator-Registered Nurse, with experience treating pediatric patients, infants and adults. Experience with diabetes, hypertension and disease control. Bilingual Spanish is a plus.


Compensation : $40 - $42

Job Function:

  • Anticipate patients needs and ensures that previsit planning is completed before visit
  • Prepares, distributes, analyzes daily patient roster
  • Provides proactive outreach to high risk patients
  • Properly documents care plans and patient interactions
  • Collaborates with Benefits Counselors and outreach staff to ensure appropriate patients scheduling and flow


Job Requirements:

  • Valid NYS RN License
  • Flu Shot
  • Covid Vaccinations
  • Bilingual Spanish
